CHIMIOTHERAPIES
I. INDICATIONS :
ATTENTION :
1. Chaque individu a une sensibilité variable face aux effets
émétogènes de la chimiothérapie et à chaque cure il faut s’adapter en
fonction de l’expérience de la précédente.
2. En cas de vomissements anticipatoires, il est nécessaire
d’envisager une prise en charge prophylactique par anxiolytique per os
type Tranxéne® ou Lysanxia® 24h av.
3. Pour les produits de risques supérieurs à 90% la prophylaxie des
nausées aiguës se fait IV puis PO pour les nausées retardées.
II. PRODUITS :
PRIMPERAN ®
Cp sécable = 10 mg
Solution buvable adulte 0.1% (1mg/ml)
Solution buvable enfant 0.4% : 10 gouttes = 1 mg
Suppo 20 mg
Suppo sécable 10 mg
1 Amp = 10 mg/2ml
Dose journalière = 0.5 mg/kg/prise 4x/jr PO ou IR
Débuter à 1 mg /kg/jr en 4 IVL
ou IVC – dose max 3 mg/kg/j
VOGALENE ®
Gel = 15 mg
Lyoc = 7.5 mg
Solution buvable adulte 0.1% (1mg/ml)
Solution buvable enfant 0.4% : 10 gouttes = 1 mg
Suppo sécable = 5 mg
Dose journalière =
15 à 30 mg/jr en 3 fois PO ou IR pour les adultes
7,5 à 15 mg/jr en 3 fois PO ou IR pour les 6 à 12 ans
1 mg/kg/jr en 3 fois PO ou IR pour les moins de 6 ans
LARGACTIL ®
solution injectable 25 mg = 5ml
solution buvable à 4% .1 goutte = 1 mg.
Dose journalière = 0,5 à 1,5 mg/kg/jr (attention
débuter à 0.5 mg/kg/jr) PO en 3 fois ou IV ds le plan des 24h
(max 40 mg/j si <23 kg et 75 mg/j si > 23 kg)
ZOPHREN ® ou autre anti 5-HT3
Faire ordonnance d'exception si sortie
Cps ou Lyoc = 4 mg
Cps ou Lyoc = 8 mg
Sirop 4mg/5ml
Amp : 2ml= 4 mg
4ml= 8 mg
Posologie =
• < 10 kg 2 mg x 2/j
• < 25 kg 4 mg x 2/j
• 25 kg 8 mg x 2/j
• 40 kg
La dose initiale habituelle est de 8 mg administrée en IV
lente 30 minutes x 2/j.
PROTOCOLE DECTANCYL®
Dexamethasone 10 mg/m2 /jr en 1 fois (max 12 mg/j) IV ou PO jusqu’au dernier jour
de chimiothérapie.
PROTOCOLE EMEND®
Indication: plus de 15 ans ou plus de 40 kg et risque émétique à 90%
