Quand peut on faire un bilan de thrombophilie chez
un(e) patient(e) atteint(e) de TVS?

Suivie pour TVS récidivantes
antécédents
- lithiases rénales bilatérales.
- céphalalgique chronique
Facteurs de risque :
– Pas de tabac,
– Pas de contraception hormonale,
Antécédents familiaux :
-Antécédent thrombotique chez la mère et chez une tante paternelle.
-Hypertension artérielle et diabète de type II chez la mère.
HDM
juillet 2022 TVS SID sans facteur favorisant -> ELIQUIS mauvaise observance récidive
douloureuse -> ELIQUIS switché par XARELTO
EVD 19 octobre 2022: récidive de TVS SID -> XARELTO switché par INNOHEP ->
amélioration douleur jambe D mais sans amélioration céphalées.
Quand peut on faire un bilan de thrombophilie chez
un(e) patient(e) atteint(e) de TVS ? suite
août 2022 bilan de thrombophilie fait par son médecin vasculaire:
hétérozygotie FV Leiden. Anticardiolipine (-), pas d’ACC
-> complément de bilan, repassage Eliquis et ETP
Complément de bilan: PS à 48 % , homocystéine normale
donc association de 2 thrombophilies héréditaires :
– hétérozygotie pour la mutation Leiden sur le gène du facteur V,
– déficit en protéine S (non génotypé),
2 conséquences :
– 2 thrombophilies + récidives de TVS dont 1 sous traitement ->
maintien TRT anticoagulant au long cours
- Informations données à la patiente pour elle-même et pour la
famille (en particulier sa plus jeune sœur )

In the patients where SVT occurred in normal veins, the
presence of FV Leiden was 26.3% of the non-spreading and
60% of the spreading to DVT; Prothrombin mutation was
found in 7.9% of the former case and in 20% of the latter;
MTHFR C677T mutation was found respectively in 23.7% and
40%. In the patients with SVT on varicose veins, the presence
of these factors was less evident (6.7%, 4.4% and 6.7%
respectively), but their prevalence was considerably higher
(35.7%, 7.4% and 21.4% respectively) in SVT spreading to DVT
than in non-spreading.

60/73 patients with SVT completed the testing protocol, while 13
dropped out; 46 of 60 patients were found to have a
thrombophilia (76.6%). The types detected were: factor V Leiden
(31/60, 51.6%), prothrombin mutation (2/60, 3.3%), MTHFR
mutation (23/60, 38.3%), antiphospholipid antibodies (5/60,
8.3%), protein C deficit (1/60, 1.6%), protein S deficit (1/60, 1.6%),
and antithrombin deficit (0/60, 0%).
Conclusions
Among patients with SVT in non-varicose veins, testing
demonstrated a high prevalence of thrombophilia. The most
common form proved to be factor V Leiden. As thrombophilia was
found to be a major cause of SVT in non-varicose veins, the
authors recommend that patients with SVT in non-varicose veins
be investigated for thrombophilia.

d’un 1er épisode de TVS non provoquée ou associée à un
contexte hormonal sur veines non variqueuses, compte
tenu de l’impact potentiel sur la prise en charge d’une

Patients with a thromboembolic complication were more
likely to have thrombophilia (11/85 vs. 3/85). The mean BMI
values in group A (25.9 ± 5.1) were significantly lower than in
group B (28.0 ± 7.2) (P = 0.034). Thromboembolic
complications were more likely to appear after foam
sclerotherapy on the lower leg (61/105) than on the thigh
(1/13) (P < 0.001), particularly after dorsal than after ventral
foam sclerotherapy (39 of 47 vs. 5 of 40, P < 0.001). Of the 39
thromboembolic complications on the dorsal lower leg, 23
were muscle vein thromboses.
Conclusion
The risk of muscle vein thrombosis after foam sclerotherapy is
especially increased in slender patients with sclerosed, dorsal
lower legs.

previous SVT alone had a 5.5-fold (4.4-6.8) increased risk of
venous thrombosis. This was 9.3 (95% CI, 7.2-12.1) combined
with a mild thrombotic risk factor, 31.4 (95% CI, 14.6-67.5)
with a strong risk factor, and 34.9 (95% CI, 19.1-63.8) in
women with a reproductive risk factor. The highest separate
risk estimates were found for SVT with surgery (42.5; 95% CI,
10.2-177.6), hospitalization (49.8; 95% CI, 11.9-209.2), or oral
contraception (43.0; 95% CI, 15.5-119.3 in women).
SVT combined with an acquired thrombotic risk factor
increases the risk of venous thrombosis 10- to 100-fold

Histoire de la maladie
TVS récidivant depuis la fin des années 90 très nombreuses récidives prédominant à gauches quelques
évènements a droite bilan complet clinique radiologique et biologique négatif en dehors
hyper homocystéinémie modérée -> B9 (malgré la supplémentation reste un peu supérieure à 10µM)
Thrombopénie modérée chronique ,
auto-immunité antithyroïdienne modérée sans hypothyroïdie avec apparition secondaire d’un FAN a un
taux faible isolé
avec montée des d- dimères aux essais d’arrêt de traitement
éveinage 2008 -> récidive -> Coumadine

- auto-immunité antithyroperoxydases à 146 U, une antithyroglobuline 51 U TSH normale
- Mise sous Xarelto aux urgences, mars 2017 surveillance D-Dimères pour arrêt: 800 -> Arixtra D-Dimères normaux avril
arrêt de l'Arixtra® en avril remontée rapide des D-Dimères à 1677 ->reprise de l'Arixtra® -> D-Dimères normaux.
- Eliquis 5 mg *2/j remontée D-Dimères (comme sous Xarelto)->Arixtra D-Dimères <500 (429 ng) maintient Arixtra
Sous Arixtra RAS sauf asthénie chronique sous B9 HCY 9,3 μmol.
Essai 'Eliquis 10 mg *2/j D-Dimères à 1000 ng -> douleurs « veineuses « des 2 membres inferieurs –>l'Arixtra5 mg D-Dimères
