RÉSUMÉ : Les AVC ischémiques sont les conséquences de pathologies CV diverses. L’enjeu du dia-
gnostic étiologique d’un AVC ou d’un AIT est majeur, car c’est de ce dernier que dépendent les
mesures de prévention secondaires et donc la réduction de la morbi-mortalité. C’est lors de la réali-
sation du bilan étiologique que la collaboration cardiologue/neurologue prend toute son importance.
L’identification d’une cause cardio-embolique implique un changement majeur de prise en charge avec
l’indication à une anticoagulation curative (par AVK ou AOD) qui s’est montrée supérieure à l’aspirine
pour diminuer les risques d’AVC cardio-emboliques sur FA (diminution du risque relatif de 68 % avec
AVK vs 21 % avec aspirine) [1-2]. Nous détaillerons ici les principales causes d’AVC ischémiques ainsi
que le bilan préconisé en première ligne, avant de s’intéresser au bilan de seconde ligne pour les
