Une proportion non négligeable des tumeurs primitivement testiculaires se développe à partir de
contingents cellulaires qui possèdent, au moins potentiellement, une capacité de synthèse hormonale.
Lorsqu’elle existe, cette sécrétion, qu’il s’agisse de stéroïdes ou de peptides, peut être responsable de
manifestations endocriniennes susceptibles de précéder de longue date l’extériorisation de la tumeur.
Parmi les trois situations cliniques qui peuvent être observées, la mise en évidence d’une masse scrotale
sans signe endocrinien d’accompagnement évident est la plus fréquente. L’enquête endocrinienne se jus-
tifie néanmoins à titre diagnostique et surtout en raison de sa valeur de marqueur pronostique et de
paramètre de suivi thérapeutique. Ailleurs, c’est le tableau endocrinien qui attire l’attention, qu’il s’agisse
d’une pseudopuberté précoce isosexuelle chez l’enfant, de signes de féminisation, de troubles érectiles ou
d’infertilité chez l’adulte. L’évaluation hormonale, dont les résultats auront une forte valeur diagnostique,
se justifie alors à la fois en raison de la nature des signes d’appel et, comme dans la situation précédente,
par sa valeur ajoutée d’indicateur pronostique et de suivi thérapeutique. Enfin, tumeur testiculaire et
signes endocriniens sont d’emblée associés. L’enquête hormonale, qui aura pour objectif de relier l’un à
l’autre, garde dans cette situation tout son intérêt pronostic et de suivi de l’efficacité du traitement.

Séminomes testiculaires
Le séminome est la plus fréquente des tumeurs testiculaires
