L’orthodontiste doit régulièrement faire
face à des patients qui consultent en
urgence, à la suite d’un traumatisme de la
sphère oro-faciale. Il rencontre alors trois
difficultés majeures :
– intégrer cette véritable urgence dans
son planning ;
– rassurer le patient et ses proches qui
affrontent une situation bouleversante ;
– bien évaluer la situation clinique. Ses
choix thérapeutiques sont décisifs : en
optant pour les procédures les mieux
adaptées, le praticien peut réduire
l’importance des séquelles fonction-
nelles et esthétiques.
Une de nos patientes, âgée de 10 ans, se
présente en urgence avec ses parents à
notre cabinet, un mercredi après midi. Elle
vient de chuter sur le menton, s’est ouvert
la lèvre, a perdu une incisive maxillaire, et
ingressé les trois autres. Une des incisives
présente une fracture coronaire avec
atteinte pulpaire.
Tout orthodontiste a vécu ce type d'aven-
ture, aussi dramatique que courante. À cet
instant précis, il lui faut puiser dans sa base
de connaissances, des réponses rapides à de
nombreuses interrogations.
– Comment s’assurer qu’aucun pro-
blème, articulaire, dentaire, radiculaire
n’est passé inaperçu ?
– Qu'est-ce qui doit être entrepris immé-

Patients who have suffered traumatic inju-
ries in the oro-facial region frequently
consult orthodontists for emergency care.
These unscheduled visits can create three
major difficulties for orthodontic offices:
– Incorporating them into an already
crowded schedule;
– Calming worried patients and their
families trying to deal with a frightening
event;
– Effectively evaluating the situation clini-
cally. The therapeutic decisions ortho-
dontists make at this crucial initial visit
will be decisive: by selecting the most
appropriate treatment plans practitio-
ners will reduce the severity of functio-
nal and esthetic sequellae to trauma.
One of our patients, a ten-year old girl,
came to see us on an emergency visit with
her parents on a Wednesday afternoon. She
had fallen down on her chin, opening up a
cut on her lip, knocking out a maxillary inci-
sor, and intruding three other teeth. One of
the incisors had a crown fracture that invol-
ved the pulp.
All orthodontists have seen this type of
accident, as dramatic as it is common. When
they do, they need to access their knowledge
base swiftly, seeking answers to a whole
range of questions, so that they can deter-
mine:
– That no problem, articular, dental, or of
the tooth roots will go unnoticed;
– Which treatments they must begin
immediately and what problems can
they safely defer to a later date?

– Y-a-t'il une indication de transplanta-
tion d'un germe de prémolaire à la
place de l’incisive manquante ?
Immédiatement dans l'alvéole béante,
ou après un certain délai ?
– Quand commencer à tracter les inci-
sives luxées ?
– Quelles mécaniques orthodontiques
employer ?
– Quel type de traitement et de recons-
truction coronaire privilégier sur l’in-
cisive fracturée ?
– En cas de fracture condylienne, com-
ment intervenir conjointement au trai-
tement maxillo-facial ?
– Quels pronostics ou informations,
donner aux parents bouleversés, quant
à la gestion d’éventuelles séquelles
faciales ou endo-buccales dans le
temps ?
– Quels sont les critères à respecter dans
la rédaction du certificat médical ?
Autant de questions qui exigent des
réponses précises, dans un délai très court.
Les informations sont difficiles à trouver
car, encore récemment, le domaine de la
traumatologie n’offrait que peu de proto-
coles définis en liaison avec un contexte
orthodontique. C'est pourquoi nous avons
souhaité faire le point des connaissances
actuelles en réunissant les avis de certains
des spécialistes les mieux informés sur ce
sujet.
Parce que les traumatismes intéressent
des tissus très différents (os, articulations,
dents, muqueuses, muscles, peau), leurs
traitements sont forcément pluridiscipli-
naires. Nous devons les aborder de manière
globale, réunir autour de nous les meilleures
compétences, et gérer le plan de traitement
comme un véritable chef d'orchestre.
C’est l'objectif de ce numéro de la Revue
d’Orthopédie Dento-Faciale d’aider le lec-
teur, dans une situation d’urgence, à
prendre les décisions les plus appropriées.

– What displaced teeth need to be put back
in proper position?
– If there will be an indication for trans-
planting a premolar tooth germ in the
place of the missing incisor, immediately
in the gaping space, or later?
– When to treat the luxated incisors?
– What orthodontic mechanics to employ?
– What will be the best approach for
repair of the fractured incisor?
– In cases of condylar fracture, how best
to intervene in cooperation with the
maxillo-facial surgeon?
– What prognosis or additional informa-
tion to provide the parents, who will
usually be in a state of shock, about what
they kind of facial and intra-oral compli-
cations they can expect to develop?
– What criteria will apply when they fill
out medical certificates?
These are a lot of demanding questions for
which orthodontists need to be prepared to
furnish precise answers in a short time. Data
in this area is hard to come by because, until
recently, the field of traumatology offered
only extremely limited protocols for ortho-
dontic issues. That is why we thought it an
appropriate task for us to sum up the current
state of knowledge on the subject and to
gather together the views of the best-infor-
med specialists in the field.
Because facial trauma affects such a
variety of tissues including bone, articula-
tions, teeth, mucous membranes, muscles,
and skin, treatment for it has to be multi-dis-
ciplinary. We should devise our treatment
plans using an over-all view, seeking out the
most competent collaborators, and then
orchestrate the team effort like veritable
maestros.
Our objective in publishing this issue of
the Revue d’Orthopédie Dento-Faciale is to
help our readers, in the emergency situations
they are bound to confront, to make the most
appropriate decisions possible.
