Dental trauma

traumatisme.
ABSTRACT
The dental pulp is frequently affected by injuries to teeth, even in the absence of overt signs
of trauma such as a coronal fracture.
Clinical and radiographic examination should not be limited to teeth that appear to be directly
damaged by the impact. Neighbouring, opposing, and even distant teeth may have been traumati-
zed by the blow, often indirectly, and should be assessed. Treatment of involved teeth begins with a
diagnosis of the condition of the pulp as well as the periodontium followed by restoration of lost
coronal, and, possibly, root structure. Finally, an orthodontic treatment approach to dealing with
these teeth based on the individual nature of the traumatic event must be formulated.

Life cycles of the pulp during the assaults to which it is subjected (Bence6):
Assaults on the pulp can lead in stages to its necrosis. For example, a healthy pulp can (a) undergo initial aggression from
the products of bacterial metabolism produced in a carious lesion (b) whose treatment can lead to partial recovery (c). But
the pulp will suffer additional insult as the dentist undertakes cavity preparation (d) and then recover to a slightly impro-
ved health status. This degree of well-being will remain unchanged (f) until the filling is replaced. At that time, the pulp
will suffer once more from an assault on its integrity (g) that will render it incapable of defending itself in the future.
Should another attack, such as trauma, ensue, degenerative changes will occur. The pulp may become painful or begin to
sink into necrosis.

This 10 year-old young girl was the victim of a traumatic blow when she was nine that left enamel fractures in her central incisors.
The teeth were restored somewhat tardily, 15 days after the accident. Two months later the patient developed an apical lesion, an abs-
cess on the upper right central incisor, which led to pulpal necrosis (c, arrow). To stimulate formation of an apical barrier, calcium
hydroxide was applied as a medication over a period of six months. Once the apical barrier had been formed (dd green arrow), the
canal was filled with gutta percha. A check-up radiograph taken three years after the procedure (e, orange arrow) showed the favo-

c d
Figures 20 a à d
dontiques.
Trauma can cause calcification of pulpal tissues that can progress to the
point where radiographs will show total closure of the root canal.
Unless an apical lesion is present, endodontic treatment is not indicated
in such cases. But the unsightly blemishes on crowns such calcification
leads to can be treated by routine, external tooth whitening (a. et b).
When a lesion exists (c, red arrow), non-surgical treatment should be
attempted first because a surgical approach to an affected apex does not
always provide access to lateral canals that may be the actual cause of
the pathosis (d, orange arrows).
