Trauma Sévère
• Définition : Lésion(s) qui menace le pronostic vital ou
dont le mécanisme ou la violence du traumatisme font
penser que de telles lésions existent (ISS > 8)
• Injury Severity Score > 15 (Σ AIS12 + AIS22 + AIS32)
• Caractéristiques :
- Lésions ne s’additionnent pas mais se multiplient
- Sous-estimation gravité lésions = piège mortel
- Oubli lésions : conséquences vitales/fonctionnelles
- Temps perdu ne se rattrape pas
• Définir priorités, hiérarchiser, organiser stratégie

Classification des Patients
• Niveau 1 : Détresse Vitale Non Stabilisée (Survie 25 %)
• PAS < 90 mmHg malgré remplissage / NADN
• SpO2 < 92 % malgré oxygénation (MHC/VM)
• HTIC persistante malgré réanimation
• Morts évitables :
- 30% auraient pu être évitées par une meilleure prise en charge
